New Patient Forms We now have our forms online for ease in processing new patients when they arrive in our office. All new patients must complete the Patient Information and the Consent Disclosure Forms. If you are authorizing the treatment of a minor child, please complete the Minor Consent Form. If you have been in an accident on the job, also complete the Workers Comp Form. Also if you have been in an auto accident, please complete the Accident Form. For your general information, we have included our Nervous System Charts, which will explain what regions of your misalignment of the spine will cause you discomfort and pain.
